Nedavno se pojavila informacija da u splitskom KBC-u žene čekaju na ultrazvuk dojke 640 dana.

Ta je informacija kod mnogih izazvala zgražanje i nevjericu, s obzirom na to da u Hrvatskoj imamo, kako političari vole reći, "jedan od najboljih zdravstvenih sustava". Što je prava istina i čeka li se zaista toliko dugo na UZV dojke, upitali smo doc. dr. sc. Krešimira Dolića, zamjenika ravnatelja KBC-a Split i predstojnika Kliničkog zavoda za dijagnostičku i intervencijsku radiologiju KBC-a Split.

– To jednostavno nije istina i žalosti me što takve pogrešne informacije stvaraju lošu sliku u javnosti. Istina je da mi na Radiologiji nastojimo zadovoljiti sve akutne potrebe za prvi pregled dojke unutar roka od mjesec dana, a u velikom broju slučajeva i uspijevamo u tome. Ako neka žena sama napipa kvržicu na dojci, pretragu joj organiziramo u roku od nekoliko dana. Sve žene koje su operirane od raka dojke imaju prednost u odnosu na sve druge i one, kad je to potrebno, vrlo brzo dobiju termin – kaže dr. Dolić.

Ipak, postoje i one koje će malo dulje pričekati UZV dojke jer ni po jednoj indikaciji ne moraju "odmah i sada" obaviti tu radiološku pretragu.

– Ako je žena starija od 50 godina i ima uredan nalaz mamografije, a ipak želi obaviti i UZV dojke, onda dolazi do određenog čekanja na pregled budući da ne postoji ni jedna opravdana indikacija da bi trebala odmah biti pregledana UZV-om.

Onkološki pacijenti ispred svih

Kad bi sve žene u Splitsko-dalmatinskoj županiji htjele obaviti UZV dojke bez obzira na to što im je mamografski nalaz uredan, tada bi 50 radiologa moralo 24 sata dnevno samo to raditi, a to nije moguće. Postoje smjernice koje moramo poštovati i sve osobe koje trebaju hitan ultrazvuk ne čekaju dulje od nekoliko dana – kaže dr. Dolić.

Evo kakva je situacija s terminima, odnosno listama čekanja i s ostalim radiološkim pretragama.

– Na MR (magnetsku rezonanciju) kontrolni pregled abdomena, zglobova, kao i na neuroradiološki pregled čeka se nešto više od godinu dana. Međutim, tu ne možemo govoriti o listi čekanja, jer ako netko ima zakazan pregled kod liječnika za godinu dana, neće valjda MR raditi, recimo, deset mjeseci prije toga.

Dakle, to nije lista čekanja, nego točno obavljena pretraga prije kontrolnog pregleda. Naravno, ako se nekome pacijentu zdravstvena situacija pogorša, njemu će prije biti obavljena MR ili neka druga pretraga. Onkološke pacijente rješavamo u najkraćem mogućem roku, oni imaju prednost u odnosu na sve ostale – kaže dr. Dolić.

Za neuro MSCT pacijent može dobiti termin u roku od 15 dana, a za MSCT abdomena čeka se od pet do šest mjeseci. Naravno, ne čekaju oni kojima je pregled hitno potreban, odnosno oni koji imaju indikaciju za hitnoću to rješavaju u roku od mjesec dana.

– Mi se na Radiologiji "rastežemo" koliko god možemo. Svaki mjesec "izmislimo" oko tisuću dodatnih termina kako bismo mogli svima izići ususret. No, veliki nam je problem i hiperinflacija indikacija, jer kolege liječnici upućuju na radiološke pretrage pacijente koji trebaju doći kod nas, ali i one koji po indikacijama ne bi trebali obaviti MR, MSCT ili neku drugu pretragu – zaključuje doc. dr. sc. Krešimir Dolić.

Na kraju je važno istaknuti da još uvijek postoji poveći broj onih koji dođu na pretragu, ali nikada ne predignu svoj nalaz, a ima i onih koji dva dana prije termina potvrde dolazak na pretragu, a ne pojave se na dan termina i na taj način "kradu" termin nekome kome bi on bio prijeko potreban. Nedavno se pojavila informacija da u splitskom KBC-u žene čekaju na ultrazvuk dojke 640 dana.

Ta je informacija kod mnogih izazvala zgražanje i nevjericu, s obzirom na to da u Hrvatskoj imamo, kako političari vole reći, "jedan od najboljih zdravstvenih sustava". Što je prava istina i čeka li se zaista toliko dugo na UZV dojke, upitali smo doc. dr. sc. Krešimira Dolića, zamjenika ravnatelja KBC-a Split i predstojnika Kliničkog zavoda za dijagnostičku i intervencijsku radiologiju KBC-a Split.

– To jednostavno nije istina i žalosti me što takve pogrešne informacije stvaraju lošu sliku u javnosti. Istina je da mi na Radiologiji nastojimo zadovoljiti sve akutne potrebe za prvi pregled dojke unutar roka od mjesec dana, a u velikom broju slučajeva i uspijevamo u tome. Ako neka žena sama napipa kvržicu na dojci, pretragu joj organiziramo u roku od nekoliko dana. Sve žene koje su operirane od raka dojke imaju prednost u odnosu na sve druge i one, kad je to potrebno, vrlo brzo dobiju termin – kaže dr. Dolić.

Ipak, postoje i one koje će malo dulje pričekati UZV dojke jer ni po jednoj indikaciji ne moraju "odmah i sada" obaviti tu radiološku pretragu.

– Ako je žena starija od 50 godina i ima uredan nalaz mamografije, a ipak želi obaviti i UZV dojke, onda dolazi do određenog čekanja na pregled budući da ne postoji ni jedna opravdana indikacija da bi trebala odmah biti pregledana UZV-om.

Onkološki pacijenti ispred svih

Kad bi sve žene u Splitsko-dalmatinskoj županiji htjele obaviti UZV dojke bez obzira na to što im je mamografski nalaz uredan, tada bi 50 radiologa moralo 24 sata dnevno samo to raditi, a to nije moguće. Postoje smjernice koje moramo poštovati i sve osobe koje trebaju hitan ultrazvuk ne čekaju dulje od nekoliko dana – kaže dr. Dolić.

Evo kakva je situacija s terminima, odnosno listama čekanja i s ostalim radiološkim pretragama.

– Na MR (magnetsku rezonanciju) kontrolni pregled abdomena, zglobova, kao i na neuroradiološki pregled čeka se nešto više od godinu dana. Međutim, tu ne možemo govoriti o listi čekanja, jer ako netko ima zakazan pregled kod liječnika za godinu dana, neće valjda MR raditi, recimo, deset mjeseci prije toga.

Dakle, to nije lista čekanja, nego točno obavljena pretraga prije kontrolnog pregleda. Naravno, ako se nekome pacijentu zdravstvena situacija pogorša, njemu će prije biti obavljena MR ili neka druga pretraga. Onkološke pacijente rješavamo u najkraćem mogućem roku, oni imaju prednost u odnosu na sve ostale – kaže dr. Dolić.

Za neuro MSCT pacijent može dobiti termin u roku od 15 dana, a za MSCT abdomena čeka se od pet do šest mjeseci. Naravno, ne čekaju oni kojima je pregled hitno potreban, odnosno oni koji imaju indikaciju za hitnoću to rješavaju u roku od mjesec dana.

– Mi se na Radiologiji "rastežemo" koliko god možemo. Svaki mjesec "izmislimo" oko tisuću dodatnih termina kako bismo mogli svima izići ususret. No, veliki nam je problem i hiperinflacija indikacija, jer kolege liječnici upućuju na radiološke pretrage pacijente koji trebaju doći kod nas, ali i one koji po indikacijama ne bi trebali obaviti MR, MSCT ili neku drugu pretragu – zaključuje doc. dr. sc. Krešimir Dolić.

Na kraju je važno istaknuti da još uvijek postoji poveći broj onih koji dođu na pretragu, ali nikada ne predignu svoj nalaz, a ima i onih koji dva dana prije termina potvrde dolazak na pretragu, a ne pojave se na dan termina i na taj način "kradu" termin nekome kome bi on bio prijeko potreban.
