Svaka stota dijagnoza karcinoma na svijetu odnosi se na multipli mijelom, teški i rijetki oblik raka krvi od kojeg obolijeva sve više mladih. Oko 1330 građana Hrvatske trenutačno se bori s ovom bolešću koja ima jedan od najdužih perioda od pojave prvih simptoma do dijagnosticiranja. Praksa je pokazala da je liječenje i do pedeset posto uspješnije ako se bolest otkrije na vrijeme, još na razini primarne zdravstvene zaštite, piše Živim.

Dosad se bolest uglavnom vezala za stariju populaciju, uz prosjek dijagnosticiranja sa 69 godina, no u posljednje vrijeme otkriva se i kod sve većeg broja mlađih pacijenata - najmlađem oboljelom u Hrvatskoj bolest je dijagnosticirana sa svega 24 godine. Naime, nespecifičnost simptoma, od kojih su najčešći umor, infekcije, anemija, bolovi u kostima i prijelomi kostiju, razlog je postavljanja dijagnoze u trenutku kada je bolest već napredovala.

Međunarodni dan multiplog mijeloma obilježava se svakog posljednjeg četvrtka u ožujku, ove godine s fokusom na individualne priče i kvalitetu života pacijenata. Pozornost se usmjerava i na prepoznavanje simptoma.

Na mijelom posumnjajte ako postoje ovi simptomi:

- dugotrajni bolovi u leđima i kostima

- opća slabost i umor

- ponavljajuće i dugotrajne infekcije (pluća, mjehura)

- povišena sedimentacija eritrocita

- oslabljena funkcija bubrega

- krvarenje iz nosa i neobjašnjive modrice

- Rano otkrivanje bolesti i što veći broj dostupnih tretmana i terapija značajno utječu na uspješnost liječenja, ali i na kvalitetu života oboljelih, budući da ovaj težak i rijedak rak krvi ne bira ni dob ni spol. Iz razgovora s liječnicima i kolegama iz regije saznajemo kako Hrvatska prednjači u dijagnostici multiplog mijeloma; u odnosu na susjedne zemlje u zadnje tri godine bilježimo i do tri puta veći broj dijagnostičkih pretraga tipičnih za dijagnozu ove opake bolesti. Isto tako, ukupni broj oboljelih u Hrvatskoj gotovo se udvostručio u odnosu na prethodno desetljeće. Razlog nije u povećanju broja oboljelih, koliko u ranijoj dijagnostici, pravovremenom početku liječenja i većem izboru dostupnih terapija - ističe Mira Armour, direktorica udruge MijelomCRO. Ipak, stručnjaci očekuju da će zbog manje učestalosti dijagnosticiranih karcinoma u pandemijskom razdoblju, rasti broj novootkrivenih.

- Zbog kvalitetnog tumačenja hematoloških nalaza, potrebna je bliska suradnja liječnika obiteljske medicine i hematologa, kao i jasan hodogram i prioritetni termini za bolesnike kod kojih sumnjamo na rak - izjavila je dr. Jadranka Karuza, specijalistica obiteljske medicine.

Prim. dr. Sandra Bašić Kinda, hematologinja na Klinici za internu medicinu KBC-a Zagreb i voditeljica Radne grupe za multipli mijelom Hrvatske kooperativne grupe za hematološke bolesti (KroHem), ističe kako danas oboljeli od multiplog mijeloma duže žive i imaju bolju kvalitetu života.

Dostupnost terapije

- Iako je u Hrvatskoj dostupnost lijekova relativno zadovoljavajuća, postoje dvije važne terapije koje nam trenutačno nisu dostupne, a zbog prirode bolesti su neophodne. To su proširenje primjene monoklonskog protutijela u prvu liniju liječenja te korištenja kombinacije monoklonskog protutijela i nove generacije inhibitora proteasoma kod bolesnika refraktornih na lenalidomid. Trenutno su obje opcije na razmatranju u HZZO-u i nadam se da će nam uskoro biti dostupne. Ove terapije još bi dodatno unaprijedile ishode, a način liječenja izjednačio se sa svim drugim zemljama EU - kaže prim. dr. Bašić Kinda.

Prof. dr. sc. Toni Valković, dr. med, hematolog sa Zavod za hematologiju Klinike za internu medicinu, KBC Rijeka ističe da se različite imunoterapije nalaze u uznapredovalim fazama kliničkih istraživanja ili su pred odobrenjem te će multipli mijelom za 10-15 godina postati izlječiva bolest za određene skupine bolesnika, a za ostale kronična bolest s kojom će se moći dugo i kvalitetno živjeti i raditi, piše Živim.

Dokumentarni film

S ciljem širenja svijesti i ključnih informacija o multiplom mijelomu, u suradnji sa srednjoeuropskim udrugama pacijenata snimljen je dokumentarni film "Moj mijelom: spremni za iduće korake“ sa svjedočanstvima oboljelih, uključujući i mladu zagrebačku obitelj. Ivanu Pravici je u 43. godini dijagnosticiran mijelom, a obitelj je nakon prvotnog straha i šoka prihvatila novu situaciju i hrabro izgradila život s mijelomom. Njihovo iskustvo i hrabra borba s ovom teškom bolesti premijerno se prikazuje na Facebook stranici Mijelom CRO, 30. ožujka na Međunarodni dan multiplog mijeloma. Osim što progovara o izazovima u liječenju i skrbi s kojima se oboljeli svakodnevno susreću, ovaj dokumentarac odaje počast svim pacijentima, njihovim obiteljima, udrugama i zdravstvenim radnicima, koji se iz dana u dan bore protiv ove opake bolesti, ne predaju i insistiraju na pozitivnom pristupu i pravu na kvalitetan i normalan život. Hrvatska premijera kompletnog filma očekuje se na jesen 2023.

Svaka stota dijagnoza karcinoma na svijetu odnosi se na multipli mijelom, teški i rijetki oblik raka krvi od kojeg obolijeva sve više mladih. Oko 1330 građana Hrvatske trenutačno se bori s ovom bolešću koja ima jedan od najdužih perioda od pojave prvih simptoma do dijagnosticiranja. Praksa je pokazala da je liječenje i do pedeset posto uspješnije ako se bolest otkrije na vrijeme, još na razini primarne zdravstvene zaštite, piše Živim.

Dosad se bolest uglavnom vezala za stariju populaciju, uz prosjek dijagnosticiranja sa 69 godina, no u posljednje vrijeme otkriva se i kod sve većeg broja mlađih pacijenata - najmlađem oboljelom u Hrvatskoj bolest je dijagnosticirana sa svega 24 godine. Naime, nespecifičnost simptoma, od kojih su najčešći umor, infekcije, anemija, bolovi u kostima i prijelomi kostiju, razlog je postavljanja dijagnoze u trenutku kada je bolest već napredovala.

Međunarodni dan multiplog mijeloma obilježava se svakog posljednjeg četvrtka u ožujku, ove godine s fokusom na individualne priče i kvalitetu života pacijenata. Pozornost se usmjerava i na prepoznavanje simptoma.

Na mijelom posumnjajte ako postoje ovi simptomi:

- dugotrajni bolovi u leđima i kostima

- opća slabost i umor

- ponavljajuće i dugotrajne infekcije (pluća, mjehura)

- povišena sedimentacija eritrocita

- oslabljena funkcija bubrega

- krvarenje iz nosa i neobjašnjive modrice

- Rano otkrivanje bolesti i što veći broj dostupnih tretmana i terapija značajno utječu na uspješnost liječenja, ali i na kvalitetu života oboljelih, budući da ovaj težak i rijedak rak krvi ne bira ni dob ni spol. Iz razgovora s liječnicima i kolegama iz regije saznajemo kako Hrvatska prednjači u dijagnostici multiplog mijeloma; u odnosu na susjedne zemlje u zadnje tri godine bilježimo i do tri puta veći broj dijagnostičkih pretraga tipičnih za dijagnozu ove opake bolesti. Isto tako, ukupni broj oboljelih u Hrvatskoj gotovo se udvostručio u odnosu na prethodno desetljeće. Razlog nije u povećanju broja oboljelih, koliko u ranijoj dijagnostici, pravovremenom početku liječenja i većem izboru dostupnih terapija - ističe Mira Armour, direktorica udruge MijelomCRO. Ipak, stručnjaci očekuju da će zbog manje učestalosti dijagnosticiranih karcinoma u pandemijskom razdoblju, rasti broj novootkrivenih.

- Zbog kvalitetnog tumačenja hematoloških nalaza, potrebna je bliska suradnja liječnika obiteljske medicine i hematologa, kao i jasan hodogram i prioritetni termini za bolesnike kod kojih sumnjamo na rak - izjavila je dr. Jadranka Karuza, specijalistica obiteljske medicine.

Prim. dr. Sandra Bašić Kinda, hematologinja na Klinici za internu medicinu KBC-a Zagreb i voditeljica Radne grupe za multipli mijelom Hrvatske kooperativne grupe za hematološke bolesti (KroHem), ističe kako danas oboljeli od multiplog mijeloma duže žive i imaju bolju kvalitetu života.

Dostupnost terapije

- Iako je u Hrvatskoj dostupnost lijekova relativno zadovoljavajuća, postoje dvije važne terapije koje nam trenutačno nisu dostupne, a zbog prirode bolesti su neophodne. To su proširenje primjene monoklonskog protutijela u prvu liniju liječenja te korištenja kombinacije monoklonskog protutijela i nove generacije inhibitora proteasoma kod bolesnika refraktornih na lenalidomid. Trenutno su obje opcije na razmatranju u HZZO-u i nadam se da će nam uskoro biti dostupne. Ove terapije još bi dodatno unaprijedile ishode, a način liječenja izjednačio se sa svim drugim zemljama EU - kaže prim. dr. Bašić Kinda.

Prof. dr. sc. Toni Valković, dr. med, hematolog sa Zavod za hematologiju Klinike za internu medicinu, KBC Rijeka ističe da se različite imunoterapije nalaze u uznapredovalim fazama kliničkih istraživanja ili su pred odobrenjem te će multipli mijelom za 10-15 godina postati izlječiva bolest za određene skupine bolesnika, a za ostale kronična bolest s kojom će se moći dugo i kvalitetno živjeti i raditi, piše Živim.

Dokumentarni film

S ciljem širenja svijesti i ključnih informacija o multiplom mijelomu, u suradnji sa srednjoeuropskim udrugama pacijenata snimljen je dokumentarni film "Moj mijelom: spremni za iduće korake“ sa svjedočanstvima oboljelih, uključujući i mladu zagrebačku obitelj. Ivanu Pravici je u 43. godini dijagnosticiran mijelom, a obitelj je nakon prvotnog straha i šoka prihvatila novu situaciju i hrabro izgradila život s mijelomom. Njihovo iskustvo i hrabra borba s ovom teškom bolesti premijerno se prikazuje na Facebook stranici Mijelom CRO, 30. ožujka na Međunarodni dan multiplog mijeloma. Osim što progovara o izazovima u liječenju i skrbi s kojima se oboljeli svakodnevno susreću, ovaj dokumentarac odaje počast svim pacijentima, njihovim obiteljima, udrugama i zdravstvenim radnicima, koji se iz dana u dan bore protiv ove opake bolesti, ne predaju i insistiraju na pozitivnom pristupu i pravu na kvalitetan i normalan život. Hrvatska premijera kompletnog filma očekuje se na jesen 2023.
