Dugo već znanstvenici sumnjaju da bi Epstein-Barrov virus (EBV), jedan od najraširenijih virusa među ljudima, mogao biti okidač za multiplu sklerozu, autoimunu bolest koja pogađa oko 36 osoba na 100.000 diljem svijeta. Ipak, nisu uspijevali objasniti kako taj virus, najpoznatiji kao uzročnik mononukleoze, u nekim slučajevima, ne kod svih zaraženih osoba, potiče čovjekov imunosni sustav da napada vlastito tkivo i to godinama nakon infekcije, piše Živim.

Sada su istraživači sa Sveučilišta Texas napravili korak naprijed u razumijevanju puta kojim virus pokreće imunosne stanice u ranim fazama bolesti, kada se javljaju prvi simptomi multiple, ali možda još nije dijagnosticirana. Kod multiple skleroze imunosni sustav pogreškom napada mijelin, zaštitnu ovojnicu koja prekriva živčana vlakna u mozgu i leđnoj moždini. Neka prethodna istraživanja sugeriraju da se to događa zbog tzv. molekularne mimikrije: proteini virusa slični su proteinima u mijelinu, pa čovjekov obrambeni sustav i domaće proteine počinje prepoznavati kao strane te kreće u napad na njih.

Zabuna obrambenog sustava

Konkretno, napad počinju antitijela koja proizvode B-stanice, vrsta bijelih krvnih stanica. Ona se zbog zabune obrambenog sustava vežu za domaće molekule i označavaju ih za uništenje. To je samo dio procesa, jer se u obranu uključuju i T-stanice, još jedna vrsta bijelih krvnih stanica. One prepoznaju alarm koji odašilju zaražene stanice, a to je pojava antigena, posebnih proteinskih fragmenata, na njihovoj površini. Tada imunosni sustav proizvodi vojsku klonova T-stanica specifičnih za taj određeni antigen.

Kako bi bolje shvatili ulogu T-stanica, znanstvenici su pratili njihovo ponašanje u uzorcima krvi i likvora (cerebrospinalne tekućine) kod osam osoba s ranim simptomima multiple, kako bi vidjeli što T-stanice prepoznaju. Usporedili su odgovore T-stanica na Epstein-Barrov virus, na laboratorijski uzgojene stanice zaražene njime, ali i na laboratorijski uzgojene stanice zaražene drugim uobičajenim virusima, poput virusa gripe. Pokazalo se da je u uzorcima krvi pacijenata 13 posto T-stanica imalo receptore za prepoznavanje stanica zaraženih EBV-om, dok je samo četiri posto prepoznalo antigene za gripu. No, u likvoru je broj T-stanica koje su prepoznale stanice zaražene EBV-om "eksplodirao" - bilo ih je čak 47 posto.

Važna uloga

Po tim rezultatima znanstvenici pretpostavljaju da T-stanice prisutne u likvoru u najranijim fazama multiple vjerojatno igraju važnu ulogu u nastanku bolesti te misle da one na neki način pridonose bolesti ili je čak uzrokuju, kako je rekao neurolog John William Lindsey, jedan od autora studije koja je objavljena u PNAS-u. Za bolje shvaćanje njihove uloge potrebna su daljnja istraživanja, svjesni su znanstvenici.

Zanimljivo je da multipla skleroza nije jedina "odgođena" bolest s kojom se povezuje Epstein-Barrov virus. Smatra se da isti uzročnik može izazvati i sindrom kroničnog umora, poznat i kao mialgični encefalomijelitis, piše Živim.

Dugo već znanstvenici sumnjaju da bi Epstein-Barrov virus (EBV), jedan od najraširenijih virusa među ljudima, mogao biti okidač za multiplu sklerozu, autoimunu bolest koja pogađa oko 36 osoba na 100.000 diljem svijeta. Ipak, nisu uspijevali objasniti kako taj virus, najpoznatiji kao uzročnik mononukleoze, u nekim slučajevima, ne kod svih zaraženih osoba, potiče čovjekov imunosni sustav da napada vlastito tkivo i to godinama nakon infekcije, piše Živim.

Sada su istraživači sa Sveučilišta Texas napravili korak naprijed u razumijevanju puta kojim virus pokreće imunosne stanice u ranim fazama bolesti, kada se javljaju prvi simptomi multiple, ali možda još nije dijagnosticirana. Kod multiple skleroze imunosni sustav pogreškom napada mijelin, zaštitnu ovojnicu koja prekriva živčana vlakna u mozgu i leđnoj moždini. Neka prethodna istraživanja sugeriraju da se to događa zbog tzv. molekularne mimikrije: proteini virusa slični su proteinima u mijelinu, pa čovjekov obrambeni sustav i domaće proteine počinje prepoznavati kao strane te kreće u napad na njih.

Zabuna obrambenog sustava

Konkretno, napad počinju antitijela koja proizvode B-stanice, vrsta bijelih krvnih stanica. Ona se zbog zabune obrambenog sustava vežu za domaće molekule i označavaju ih za uništenje. To je samo dio procesa, jer se u obranu uključuju i T-stanice, još jedna vrsta bijelih krvnih stanica. One prepoznaju alarm koji odašilju zaražene stanice, a to je pojava antigena, posebnih proteinskih fragmenata, na njihovoj površini. Tada imunosni sustav proizvodi vojsku klonova T-stanica specifičnih za taj određeni antigen.

Kako bi bolje shvatili ulogu T-stanica, znanstvenici su pratili njihovo ponašanje u uzorcima krvi i likvora (cerebrospinalne tekućine) kod osam osoba s ranim simptomima multiple, kako bi vidjeli što T-stanice prepoznaju. Usporedili su odgovore T-stanica na Epstein-Barrov virus, na laboratorijski uzgojene stanice zaražene njime, ali i na laboratorijski uzgojene stanice zaražene drugim uobičajenim virusima, poput virusa gripe. Pokazalo se da je u uzorcima krvi pacijenata 13 posto T-stanica imalo receptore za prepoznavanje stanica zaraženih EBV-om, dok je samo četiri posto prepoznalo antigene za gripu. No, u likvoru je broj T-stanica koje su prepoznale stanice zaražene EBV-om "eksplodirao" - bilo ih je čak 47 posto.

Važna uloga

Po tim rezultatima znanstvenici pretpostavljaju da T-stanice prisutne u likvoru u najranijim fazama multiple vjerojatno igraju važnu ulogu u nastanku bolesti te misle da one na neki način pridonose bolesti ili je čak uzrokuju, kako je rekao neurolog John William Lindsey, jedan od autora studije koja je objavljena u PNAS-u. Za bolje shvaćanje njihove uloge potrebna su daljnja istraživanja, svjesni su znanstvenici.

Zanimljivo je da multipla skleroza nije jedina "odgođena" bolest s kojom se povezuje Epstein-Barrov virus. Smatra se da isti uzročnik može izazvati i sindrom kroničnog umora, poznat i kao mialgični encefalomijelitis, piše Živim.
