Epidemija prekomjernih dijagnoza karcinoma štitnjače se brzo širi u svijetu i zahvatila je više od milijun osoba u tridesetak zemalja, prema novom istraživanju Agencije za rak Svjetske zdravstvene organizacije (WHO).

Rak štitnjače je među onim bolestima kod kojih je "predijagnosticiranje" često, pa se i agresivno liječe iako ne postoji visoki rizik za zdravlje. O predijagnosticiranju se govori kada se ljudima bez ikakvih tegoba dijagnosticira neki problem, premda im on neće stvoriti nikakve smetnje, niti će od njega ranije umrijeti.

Predijagnosticiranjem raka štitnjače uglavnom su pogođene žene srednje dobi. Istraživanje objavljeno u medicinskom časopisu The Lancet Diabetes & Endocrinology vođeno je u 26 zemalja, a proveli su ga znanstvenici iz Međunarodnog centra za istraživanje karcinoma (EACR) sa sjedištem u Lyonu u suradnji s Nacionalnim institutom za rak u Avianu u Italiji.

Više od milijun osoba (830.000 žena i 220.000 muškaraca) dobilo je dijagnozu karcinoma štitnjače između 2008. i 2012. u tih 26 zemalja. Procijenjeni udio slučajeva karcinoma štitnjače kod žena koji se može pripisati nepotrebnoj dijagnostici između 2008. i 2012. iznosi oko 93 posto u Južnoj Koreji, 91 posto u Bjelorusiji, 87 posto u Kini, 84 posto u Italiji i u Hrvatskoj te 83 posto u Slovačkoj i Francuskoj.

Tristo devedeset tisuća žena u Kini, 140.000 u Južnoj Koreji, 120.000 u SAD-u, 31.000 u Italiji i 25.000 žena u Francuskoj dobilo je između 2008. i 2012. nepotrebnu dijagnozu raka štitnjače, iako bi normalno s njim nastavile živjeti.

Zbog pretjerane dijagnostike dolazi do uklanjanja kompletne štitnjače ili doživotnog tretmana koji možda nije potreban.

Rezultati istraživanja "upućuju na to da je velika većina dijagnoza karcinoma štitnjače posljedica pretjerane dijagnostike", rekao je Salvatore Vaccarella s EACR-a koji je vodio istraživanje. On je naglasio da je hitno potrebno pomno pratiti globalni razvoj dijagnostike zbog njegovih razmjera i utjecaja, a nedavne smjernice izričito preporučuju da se ne radi dijagnostika eventualnog karcinoma štitnjače kod osoba koje nemaju simptome.

Komentar stručnjaka

Najveći napredak medicine temelji se na brzom napretku tehnologije. Tako danas postoje suvremene metode dijagnostike različitih sustava pa tako i UZV štitnjače koji detektira jako male promjene u tkivu štitnjače. Takve promjene podliježu daljnjoj dijagnostici: citološkoj punkciji, scintigrafiji, određivanju razine hormona štitnjače, antitijela te eventualnoj daljnjoj endokrinološkoj i imunološkoj dijagnostici.

Svaki karcinom se zove upravo tako jer je opasan ako se pravodobno i adekvatno ne liječi. Također može stvarati metastaze kako lokalne tako i udaljene što bolest dodatno pogoršava kao i ishode liječenja. Ovome još možemo dodati faktore stresa, različite štetne egzogene i endogene utjecaje koji su sve viže izraženi i mogu dovesti do malignih promjena različitih organa pa tako i štitnjače koja je posebno osjetljiva. Živimo u suvremenom svijetu, u skladu sa suvremenim postulatima struke i znanosti pa bi se tako trebali ponašati i djelovati. Rana dijagnostika = rano izlječenje i izbjegavanje daljnjih komplikacija, a ujedno i troškova liječenja. Budimo svi odgovorni da bi bili što zdraviji! - kaže nam splitska otorinolaringologinja.

Epidemija prekomjernih dijagnoza karcinoma štitnjače se brzo širi u svijetu i zahvatila je više od milijun osoba u tridesetak zemalja, prema novom istraživanju Agencije za rak Svjetske zdravstvene organizacije (WHO).

Rak štitnjače je među onim bolestima kod kojih je "predijagnosticiranje" često, pa se i agresivno liječe iako ne postoji visoki rizik za zdravlje. O predijagnosticiranju se govori kada se ljudima bez ikakvih tegoba dijagnosticira neki problem, premda im on neće stvoriti nikakve smetnje, niti će od njega ranije umrijeti.

Predijagnosticiranjem raka štitnjače uglavnom su pogođene žene srednje dobi. Istraživanje objavljeno u medicinskom časopisu The Lancet Diabetes & Endocrinology vođeno je u 26 zemalja, a proveli su ga znanstvenici iz Međunarodnog centra za istraživanje karcinoma (EACR) sa sjedištem u Lyonu u suradnji s Nacionalnim institutom za rak u Avianu u Italiji.

Više od milijun osoba (830.000 žena i 220.000 muškaraca) dobilo je dijagnozu karcinoma štitnjače između 2008. i 2012. u tih 26 zemalja. Procijenjeni udio slučajeva karcinoma štitnjače kod žena koji se može pripisati nepotrebnoj dijagnostici između 2008. i 2012. iznosi oko 93 posto u Južnoj Koreji, 91 posto u Bjelorusiji, 87 posto u Kini, 84 posto u Italiji i u Hrvatskoj te 83 posto u Slovačkoj i Francuskoj.

Tristo devedeset tisuća žena u Kini, 140.000 u Južnoj Koreji, 120.000 u SAD-u, 31.000 u Italiji i 25.000 žena u Francuskoj dobilo je između 2008. i 2012. nepotrebnu dijagnozu raka štitnjače, iako bi normalno s njim nastavile živjeti.

Zbog pretjerane dijagnostike dolazi do uklanjanja kompletne štitnjače ili doživotnog tretmana koji možda nije potreban.

Rezultati istraživanja "upućuju na to da je velika većina dijagnoza karcinoma štitnjače posljedica pretjerane dijagnostike", rekao je Salvatore Vaccarella s EACR-a koji je vodio istraživanje. On je naglasio da je hitno potrebno pomno pratiti globalni razvoj dijagnostike zbog njegovih razmjera i utjecaja, a nedavne smjernice izričito preporučuju da se ne radi dijagnostika eventualnog karcinoma štitnjače kod osoba koje nemaju simptome.

Komentar stručnjaka

Najveći napredak medicine temelji se na brzom napretku tehnologije. Tako danas postoje suvremene metode dijagnostike različitih sustava pa tako i UZV štitnjače koji detektira jako male promjene u tkivu štitnjače. Takve promjene podliježu daljnjoj dijagnostici: citološkoj punkciji, scintigrafiji, određivanju razine hormona štitnjače, antitijela te eventualnoj daljnjoj endokrinološkoj i imunološkoj dijagnostici.

Svaki karcinom se zove upravo tako jer je opasan ako se pravodobno i adekvatno ne liječi. Također može stvarati metastaze kako lokalne tako i udaljene što bolest dodatno pogoršava kao i ishode liječenja. Ovome još možemo dodati faktore stresa, različite štetne egzogene i endogene utjecaje koji su sve viže izraženi i mogu dovesti do malignih promjena različitih organa pa tako i štitnjače koja je posebno osjetljiva. Živimo u suvremenom svijetu, u skladu sa suvremenim postulatima struke i znanosti pa bi se tako trebali ponašati i djelovati. Rana dijagnostika = rano izlječenje i izbjegavanje daljnjih komplikacija, a ujedno i troškova liječenja. Budimo svi odgovorni da bi bili što zdraviji! - kaže nam splitska otorinolaringologinja.

Epidemija prekomjernih dijagnoza karcinoma štitnjače se brzo širi u svijetu i zahvatila je više od milijun osoba u tridesetak zemalja, prema novom istraživanju Agencije za rak Svjetske zdravstvene organizacije (WHO).

Rak štitnjače je među onim bolestima kod kojih je "predijagnosticiranje" često, pa se i agresivno liječe iako ne postoji visoki rizik za zdravlje. O predijagnosticiranju se govori kada se ljudima bez ikakvih tegoba dijagnosticira neki problem, premda im on neće stvoriti nikakve smetnje, niti će od njega ranije umrijeti.

Predijagnosticiranjem raka štitnjače uglavnom su pogođene žene srednje dobi. Istraživanje objavljeno u medicinskom časopisu The Lancet Diabetes & Endocrinology vođeno je u 26 zemalja, a proveli su ga znanstvenici iz Međunarodnog centra za istraživa...
