Poznato je da je česta uporaba lijekova protiv bolova problematična. Sada su, međutim, istraživači otkrili da može imati čak i paradoksalan učinak. Dok lijekovi protiv bolova mogu pomoći u ublažavanju akutne boli, oni također povećavaju rizik od kronične boli. Nalaz se odnosi na tzv. protuupalne lijekove protiv bolova, koji uključuju aktivne sastojke kao što su aspirin i ibuprofen. Akutni upalni odgovor je neugodan, ali može biti koristan - između ostalog i za sprječavanje kronične boli.

Prvi dio studije uključivao je ljude s bolovima u donjem dijelu leđa. Studija koja rasvjetljava vezu između upotrebe protuupalnih lijekova protiv bolova i povećanog rizika od razvoja kronične boli nedavno je objavljena u časopisu Science Translational Medicine. U prvom dijelu istraživanja sudjelovalo je 98 osoba koje su se borile s bolovima u donjem dijelu leđa. Tri mjeseca su primali protuupalne lijekove protiv bolova za ublažavanje bolova. Istraživači su željeli proučiti mehanizme koji stoje iza osjeta boli, i što je najvažnije, učinak protuupalnih lijekova na bol. Na kraju tromjesečnog razdoblja formirane su dvije grupe. Prvu skupinu činili su ljudi kod kojih su protuupalni lijekovi protiv bolova ublažavali bol, a drugu skupinu činili su ljudi kojima lijekovi nisu pomogli. Istraživači su odabrali isti broj ljudi iz obje skupine, zatim su proučavali njihov genom i usporedili rezultate obiju skupina.

Učinkovitost lijekova usko je povezana s neutrofilima

Najvažnija razlika između genoma prve i druge skupine odnosila se na neutrofile ili neutrofilne granulocite, koji su jedan od podtipova bijelih krvnih stanica - stanica poznatih i kao leukociti, a koje su iznimno važne za adekvatan imunološki odgovor. Neutrofili su posebno važni u ranim fazama upale. Naime, daju bazu koja omogućuje obnovu oštećenih tkiva. Bolesnici liječeni protuupalnim lijekovima protiv bolova imali su vrlo visok broj neutrofila prije liječenja, mnogo veći nego u drugoj skupini, a uočen je brz pad tijekom tri mjeseca primjene protuupalnih lijekova protiv bolova. Slična promjena je također uočena u studiji koja je uključivala 30 ljudi s temporomandibularnim poremećajem koji uzrokuje jake bolove u području čeljusti i ušiju. To je potvrdilo da su neutrofili vrlo važni u ublažavanju boli.

Blokada neutrofila i protuupalni lijekovi protiv bolova - nepovoljna kombinacija

Istraživači su tada otkrili da blokada neutrofila može značajno produljiti razdoblje percepcije boli. Međutim, od posebne je važnosti otkriće da ako su neutrofili blokirani, protuupalni lijekovi protiv bolova mogu učiniti više štete nego koristi. Razdoblje oporavka može biti dulje nego što bi bilo da se lijekovi protiv bolova ne koriste. To je zbog protuupalnog djelovanja nekih lijekova protiv bolova. Sličan odgovor nije se mogao otkriti s lijekovima protiv bolova bez protuupalnog učinka.

Povećan rizik od kronične boli u budućnosti

Uslijedilo je istraživanje podataka za čak pola milijuna ljudi prikupljenih u sklopu projekta UK Biobank. Istraživači su otkrili da su oni koji su liječeni protuupalnim lijekovima protiv bolova za akutnu bol imali dvostruko veću vjerojatnost da će imati kroničnu bol 2-10 godina nakon liječenja od onih koji su koristili za liječenje akutne boli druga sredstva ili metode. Istraživači su upozorili da je prerano savjetovati potpuno napuštanje protuupalnih lijekova protiv bolova. Između ostalog, bit će potrebne kliničke studije za izravnu usporedbu učinaka protuupalnih lijekova i drugih lijekova protiv bolova koji se ne temelje na protuupalnom djelovanju.

Poznato je da je česta uporaba lijekova protiv bolova problematična. Sada su, međutim, istraživači otkrili da može imati čak i paradoksalan učinak. Dok lijekovi protiv bolova mogu pomoći u ublažavanju akutne boli, oni također povećavaju rizik od kronične boli. Nalaz se odnosi na tzv. protuupalne lijekove protiv bolova, koji uključuju aktivne sastojke kao što su aspirin i ibuprofen. Akutni upalni odgovor je neugodan, ali može biti koristan - između ostalog i za sprječavanje kronične boli.

Prvi dio studije uključivao je ljude s bolovima u donjem dijelu leđa. Studija koja rasvjetljava vezu između upotrebe protuupalnih lijekova protiv bolova i povećanog rizika od razvoja kronične boli nedavno je objavljena u časopisu Science Translational Medicine. U prvom dijelu istraživanja sudjelovalo je 98 osoba koje su se borile s bolovima u donjem dijelu leđa. Tri mjeseca su primali protuupalne lijekove protiv bolova za ublažavanje bolova. Istraživači su željeli proučiti mehanizme koji stoje iza osjeta boli, i što je najvažnije, učinak protuupalnih lijekova na bol. Na kraju tromjesečnog razdoblja formirane su dvije grupe. Prvu skupinu činili su ljudi kod kojih su protuupalni lijekovi protiv bolova ublažavali bol, a drugu skupinu činili su ljudi kojima lijekovi nisu pomogli. Istraživači su odabrali isti broj ljudi iz obje skupine, zatim su proučavali njihov genom i usporedili rezultate obiju skupina.

Učinkovitost lijekova usko je povezana s neutrofilima

Najvažnija razlika između genoma prve i druge skupine odnosila se na neutrofile ili neutrofilne granulocite, koji su jedan od podtipova bijelih krvnih stanica - stanica poznatih i kao leukociti, a koje su iznimno važne za adekvatan imunološki odgovor. Neutrofili su posebno važni u ranim fazama upale. Naime, daju bazu koja omogućuje obnovu oštećenih tkiva. Bolesnici liječeni protuupalnim lijekovima protiv bolova imali su vrlo visok broj neutrofila prije liječenja, mnogo veći nego u drugoj skupini, a uočen je brz pad tijekom tri mjeseca primjene protuupalnih lijekova protiv bolova. Slična promjena je također uočena u studiji koja je uključivala 30 ljudi s temporomandibularnim poremećajem koji uzrokuje jake bolove u području čeljusti i ušiju. To je potvrdilo da su neutrofili vrlo važni u ublažavanju boli.

Blokada neutrofila i protuupalni lijekovi protiv bolova - nepovoljna kombinacija

Istraživači su tada otkrili da blokada neutrofila može značajno produljiti razdoblje percepcije boli. Međutim, od posebne je važnosti otkriće da ako su neutrofili blokirani, protuupalni lijekovi protiv bolova mogu učiniti više štete nego koristi. Razdoblje oporavka može biti dulje nego što bi bilo da se lijekovi protiv bolova ne koriste. To je zbog protuupalnog djelovanja nekih lijekova protiv bolova. Sličan odgovor nije se mogao otkriti s lijekovima protiv bolova bez protuupalnog učinka.

Povećan rizik od kronične boli u budućnosti

Uslijedilo je istraživanje podataka za čak pola milijuna ljudi prikupljenih u sklopu projekta UK Biobank. Istraživači su otkrili da su oni koji su liječeni protuupalnim lijekovima protiv bolova za akutnu bol imali dvostruko veću vjerojatnost da će imati kroničnu bol 2-10 godina nakon liječenja od onih koji su koristili za liječenje akutne boli druga sredstva ili metode. Istraživači su upozorili da je prerano savjetovati potpuno napuštanje protuupalnih lijekova protiv bolova. Između ostalog, bit će potrebne kliničke studije za izravnu usporedbu učinaka protuupalnih lijekova i drugih lijekova protiv bolova koji se ne temelje na protuupalnom djelovanju.
