Maligni melanom je tumor melanocita i najzloćudniji je tumor kože i sluznica. Njegovo biološko zloćudno ponašanje se ne očituje toliko u njegovoj lokalnoj agresivnosti na koži koliko u njegovoj upadljivoj sklonosti ranom limfogenom ili hematogenom metastaziranju u unutarnje organe oboljele osobe, piše Sv. Katarina.

Učestalost javljanja melanoma

Maligni melanom se pojavljuje pretežno kod bijelaca. Njegova se učestalost pojavljuje i raste intenzivno u cijelom svijetu. U zemljama EU gdje i Hrvatska pripada učestalost je 14/100.000 stanovnika.

U Europi se dva puta češće javlja kod žena nego muškaraca. Postoji i genetski predispozicija za pojavu melanoma. U 1 do 7% bolesnika s melanomom uočeno je obiteljsko pojavljivanje tog tumora. Pomišlja se na autonomnost dominantno ili poligenski način nasljedovanja.

Utjecaj čimbenika iz okoliša

Poznato je da je neprijeporno utjecaj ultraljubičastog zračenja i ultraljubičastih zraka.

Kod dugotrajnog i nekontroliranog izlaganja suncu uz nastanke opeklina u ranoj životnoj dobi, još u djetinjstvu utječe na pojavu malignog melanoma kasnije u životu. Nažalost, maligni melanom se javlja već kod mladih ljudi i to je vrlo zabrinjavajući faktor.

Osobito su ugrožene osobe svijetle puti, svijetlih plavih ili zelenih očiju, plave ili crvenkaste kose koje su bile učestalo izlagane djelovanju Sunčeve svijetla. Zemlje bliže ekvatoru zbog većeg broja sunčanih dana u godini imaju i znatno veći broj oboljelih.

Etiopatogeneza malignog melanoma

Sunčevo je svijetlo najbolje istraženi čimbenik rizika u nastanku malignog melanoma. UVA zrake sunca kumuliraju se u koži od rođenja.

Drugi važan čimbenik je postojanje melanocitnih lezija koje mogu evoluirati u maligni melanom kao što su displastični ili već promijenjeni madeži te kongenitalni ili urođeni madeži. Treći čimbenik je genetski predispozicija za nastanak malignog melanoma. Na kraju treba naglasiti da imunosupresivna terapija kod transplantacije organa ili ona stečena imunosupresija (AIDS) koje su također jedan od čimbenika nastanka malignog melanoma.

Osobina malignog melanoma je sklonost metastaziranju putem limfe ili krvlju te mogu otići u bilo koji organ u tijelu u vidu metastaze.

U 50% slučajeva maligni melanom nastaje na klinički nepromijenjenom koži, često vrlo mali, dok se oko 40% malignih melanoma razvija iz displastičnih madeža ili iz kongenitalnih madeža.

Obično se radi o ravnim madežima, nedavnih rubova, različitog diskolorita tamno smeđe do crne boje. Često se pojavljuju u foto eksponirani mjestima.

Postoji nekoliko tipova malignih melanoma:

Lentigo maligna melanoma LMM) - javlja se u 5 - 30% pacijenata, najčešće na fotoeksponiranim mjestima na tijelu

Površinsko šireći melanom - superficial spreading melanoma (SSM) čija učestalost je 70%, osobito na koži dosta izlaganja Sunčevoj svjetlosti

Nodularni melanom kojeg obilježava vrlo brz razvitak te je i najagresivniji

Melanoma maligni acrolentiginosum (ALM) – maligni melanom koji se razvija u predjelu dlanova i tabana te ispod noktiju na prstima i na sluznicama

Amelanotični maligni melanom (AMM) kojeg je teško dijagnosticirati. Na njega treba pomišljati kod erozičnih tumora dlanova i tabana. Nije još razjašnjeno zašto tumorske stanice ne stvaraju melanin, a ovaj amelanotični melanom je daleko agresivniji od pigmentnih malignih melanoma

Terapija malignih melanoma je njihova široka ekscizija te PHD analiza kojom saznajemo o kojem se tipu melanoma radi i o njegovoj agresivnosti.

Potrebna je kompletna obrada pacijenta da se isključe ili nađu metastaze te se dalje tretiraju prema protokolu.

Prevencija nastanka melanoma

Najbolja prevencija nastanka malignog melanoma je zaštita od UVA i UVB zraka Sunčevog spektra s visokim SPF 50+ kod izlaganja suncu od proljeća pa tijekom cijelog ljeta bez obzira bili na moru ili u gradu.

Bitno je nošenje pokrivala za glavu te naočala za sunce kod boravka na suncu, a preporučuje se boraviti na suncu ujutro do 10 sati, a popodne od 17 sati pa nadalje.

Madeže je potrebno pregledavati specijalnim uređajem, dermatoskopom kod dermatovenerologa jednom godišnje i to madeže na cijelom tijelu jer melanom ne bira mjesto te može biti pacijentu posve neprimjetan.

Ako postoji genetska predispozicija za nastanak melanoma, kompletni pregledi madeža se preporučuju činiti 2 x godišnje. Ako pacijent osjeti svrbež, peckanja ili krvarenje madeža, nužno je odmah se javiti na pregled dermatovenerologu i utvrditi radi li se o malignom melanomu te učiniti uklanjanje istoga te uzorak poslati na PHD obradu.

Važna je briga svakog pojedinca za sebe i svoje zdravlje pa tako ne bi trebao biti ni problem povremeno napraviti samopregled madeža. Kada postoji sumnja da nešto nije u redu s madežom, uvijek je bolje pregledati samo taj madež i intervenirati ako treba u vidu ekscizije, a ako nije potrebno pacijent je barem siguran i miran.

Maligni melanom je tumor melanocita i najzloćudniji je tumor kože i sluznica. Njegovo biološko zloćudno ponašanje se ne očituje toliko u njegovoj lokalnoj agresivnosti na koži koliko u njegovoj upadljivoj sklonosti ranom limfogenom ili hematogenom metastaziranju u unutarnje organe oboljele osobe, piše Sv. Katarina.

Učestalost javljanja melanoma

Maligni melanom se pojavljuje pretežno kod bijelaca. Njegova se učestalost pojavljuje i raste intenzivno u cijelom svijetu. U zemljama EU gdje i Hrvatska pripada učestalost je 14/100.000 stanovnika.

U Europi se dva puta češće javlja kod žena nego muškaraca. Postoji i genetski predispozicija za pojavu melanoma. U 1 do 7% bolesnika s melanomom uočeno je obiteljsko pojavljivanje tog tumora. Pomišlja se na autonomnost dominantno ili poligenski način nasljedovanja.

Utjecaj čimbenika iz okoliša

Poznato je da je neprijeporno utjecaj ultraljubičastog zračenja i ultraljubičastih zraka.

Kod dugotrajnog i nekontroliranog izlaganja suncu uz nastanke opeklina u ranoj životnoj dobi, još u djetinjstvu utječe na pojavu malignog melanoma kasnije u životu. Nažalost, maligni melanom se javlja već kod mladih ljudi i to je vrlo zabrinjavajući faktor.

Osobito su ugrožene osobe svijetle puti, svijetlih plavih ili zelenih očiju, plave ili crvenkaste kose koje su bile učestalo izlagane djelovanju Sunčeve svijetla. Zemlje bliže ekvatoru zbog većeg broja sunčanih dana u godini imaju i znatno veći broj oboljelih.

Etiopatogeneza malignog melanoma

Sunčevo je svijetlo najbolje istraženi čimbenik rizika u nastanku malignog melanoma. UVA zrake sunca kumuliraju se u koži od rođenja.

Drugi važan čimbenik je postojanje melanocitnih lezija koje mogu evoluirati u maligni melanom kao što su displastični ili već promijenjeni madeži te kongenitalni ili urođeni madeži. Treći čimbenik je genetski predispozicija za nastanak malignog melanoma. Na kraju treba naglasiti da imunosupresivna terapija kod transplantacije organa ili ona stečena imunosupresija (AIDS) koje su također jedan od čimbenika nastanka malignog melanoma.

Osobina malignog melanoma je sklonost metastaziranju putem limfe ili krvlju te mogu otići u bilo koji organ u tijelu u vidu metastaze.

U 50% slučajeva maligni melanom nastaje na klinički nepromijenjenom koži, često vrlo mali, dok se oko 40% malignih melanoma razvija iz displastičnih madeža ili iz kongenitalnih madeža.

Obično se radi o ravnim madežima, nedavnih rubova, različitog diskolorita tamno smeđe do crne boje. Često se pojavljuju u foto eksponirani mjestima.

Postoji nekoliko tipova malignih melanoma:

Lentigo maligna melanoma LMM) - javlja se u 5 - 30% pacijenata, najčešće na fotoeksponiranim mjestima na tijelu

Površinsko šireći melanom - superficial spreading melanoma (SSM) čija učestalost je 70%, osobito na koži dosta izlaganja Sunčevoj svjetlosti

Nodularni melanom kojeg obilježava vrlo brz razvitak te je i najagresivniji

Melanoma maligni acrolentiginosum (ALM) – maligni melanom koji se razvija u predjelu dlanova i tabana te ispod noktiju na prstima i na sluznicama

Amelanotični maligni melanom (AMM) kojeg je teško dijagnosticirati. Na njega treba pomišljati kod erozičnih tumora dlanova i tabana. Nije još razjašnjeno zašto tumorske stanice ne stvaraju melanin, a ovaj amelanotični melanom je daleko agresivniji od pigmentnih malignih melanoma

Terapija malignih melanoma je njihova široka ekscizija te PHD analiza kojom saznajemo o kojem se tipu melanoma radi i o njegovoj agresivnosti.

Potrebna je kompletna obrada pacijenta da se isključe ili nađu metastaze te se dalje tretiraju prema protokolu.

Prevencija nastanka melanoma

Najbolja prevencija nastanka malignog melanoma je zaštita od UVA i UVB zraka Sunčevog spektra s visokim SPF 50+ kod izlaganja suncu od proljeća pa tijekom cijelog ljeta bez obzira bili na moru ili u gradu.

Bitno je nošenje pokrivala za glavu te naočala za sunce kod boravka na suncu, a preporučuje se boraviti na suncu ujutro do 10 sati, a popodne od 17 sati pa nadalje.

Madeže je potrebno pregledavati specijalnim uređajem, dermatoskopom kod dermatovenerologa jednom godišnje i to madeže na cijelom tijelu jer melanom ne bira mjesto te može biti pacijentu posve neprimjetan.

Ako postoji genetska predispozicija za nastanak melanoma, kompletni pregledi madeža se preporučuju činiti 2 x godišnje. Ako pacijent osjeti svrbež, peckanja ili krvarenje madeža, nužno je odmah se javiti na pregled dermatovenerologu i utvrditi radi li se o malignom melanomu te učiniti uklanjanje istoga te uzorak poslati na PHD obradu.

Važna je briga svakog pojedinca za sebe i svoje zdravlje pa tako ne bi trebao biti ni problem povremeno napraviti samopregled madeža. Kada postoji sumnja da nešto nije u redu s madežom, uvijek je bolje pregledati samo taj madež i intervenirati ako treba u vidu ekscizije, a ako nije potrebno pacijent je barem siguran i miran.
