Inhibitori fosfodiesteraze tipa 5 (PDE5i) – lijek za erektilnu disfunkciju koji se prodaje pod nazivima Viagra, Levitra, Cialis i drugi – uobičajeni su medicinski tretmani za erektilnu disfunkciju (ED). Međutim, nova švedska studija objavljena u „Journal of the American College of Cardiology“ ukazuje na to da su pacijenti izloženi većem riziku od kardioloških oboljenja tijekom vremena kada im se propisuju PDE5 i nitratni lijekovi.

Erektilna disfunkcija je uobičajeno stanje kod srednjovjekovnih i starijih muškaraca i snažan je prediktor koronarne arterijske bolesti. Nitrati su lijekovi koji se obično koriste za liječenje angine ili bolova u grudima, prenosi Kurir.

Oba mogu izazvati pad krvnog pritiska, pa su kontraindicirani za zajedničku upotrebu. Međutim, malo je podataka u stvarnom svijetu o implikacijama upotrebe oba i broj ljudi kojima je propisano i jedno i drugo raste.

Švedski znanstvenici proučavali su 61.400 muškaraca na nitratima, od kojih je 5.700 također koristilo tablete za erektilnu disfunkciju. Muškarci koji su uzimali oboje imali su ukupni veći rizik od smrti plus srčani udar, zatajenje srca i druge srčane probleme.

Malo ih se dogodilo unutar 28 dana nakon što su muškarci primili lijek za erektilnu disfunkciju, što ukazuje da postoji nizak neposredni rizik.

Ograničenja studije uključuju nemogućnost saznanja o suradljivosti pacijenata i navikama uzimanja lijekova te nemogućnost zaključivanja uzroka smrti iz podataka. Istraživači su procijenili upotrebu prema dobivenim receptima, ali nisu znali koliko su pacijenti bili sukladni ili kakve su bile njihove navike uzimanja lijekova.

Također, populacija pacijenata uključivala je visokorizične osobe koje su već imale MI ili revaskularizaciju. Također su im propisani nitrati najmanje dva puta i, unatoč preporukama smjernica, također su im propisani PDE5i najmanje dva puta; stoga se rezultati možda neće moći u potpunosti generalizirati na opću populaciju.

Potrebna su daljnja istraživanja kako bi se u potpunosti razumjeli učinci kombinacije tretmana.

Također, istraživači su rekli da su kod pacijenata s ishemijskom bolesti srca i samo blagom anginom s razumnim kapacitetom vježbanja, ED PDE5i prilično sigurni - ako pacijent nije na kroničnoj terapiji nitratima. Međutim, kod onih koji su na kroničnoj oralnoj terapiji nitratima, upotreba PDE5i je u najboljem slučaju nepreporučljiva i općenito kontraindicirana.

Ograničenja studije uključuju nemogućnost saznanja o suradljivosti pacijenata i navikama uzimanja lijekova te nemogućnost zaključivanja uzroka smrti iz podataka.
