‘Moj suprug je prije šest mjeseci imao infarkt miokarda. Uspješno mu je ugrađen stent. Uzima puno tableta u određeno vrijeme tijekom dana. Dobro se osjeća, osim što mu se pojavila erektilna disfunkcija. Prije infarkta sve je bilo u redu. Pretpostavljam da je to posljedica uzimanja određenih lijekova (za razrjeđivanje krvi).

Što je uzrok takve disfunkciji i jesu li to lijekovi koje uzima? Ruke i noge su mu hladne, mislim da to upućuje na lošu cirkulaciju. Hoće li se to moći popraviti i je li se s vremenom smanjuje broj lijekova kod takve dijagnoze?‘, upitala je gospođa iz Krapine, a odgovara dr. Dubravka Šušnjar, internist-kardiolog, KB Dubrava.

‘Budući da je vaš muž nedavno imao infarkt miokarda, uspješno je liječen perkutanom koronarnom intervencijom s ugradnjom stenta i nakon toga je uvedena medikamentozna terapija.

Uzimanje optimalne medikamentozne terapije nakon akutnog koronarnog sindroma potrebno je radi sprečavanja ponovnog ishemijskog događaja, poboljšanja srčane funkcije te poboljšanja preživljenja, stoga je nužno da se bolesnici toga pridržavaju.

Pri tome su antihipertenzivi najčešće izolirana skupina, a beta-blokatori i tiazidski diuretici prema brojnim studijama najviše povećavaju rizik od erektilne disfunkcije.

Točan mehanizam pojave erektilne disfunkcije uzrokovane beta-blokatorima nije sasvim razjašnjen. Smatra se da je to posljedica učinka na adrenergički sustav koji je uključen u fazu erekcije. Zanimljivo je da je jedna recentna studija pokazala da uloga periferne vazokonstrikcije kao posljedica uzimanja beta-blokatora ipak nema značajan učinak na erektilnu funkciju.

Ako se bolesnik pridržava svih savjeta liječnika, izbjegava čimbenike rizika za nastanak kardiovaskularnih događaja, uzima svu propisanu terapiju te se na kontrolnom kardiološkom pregledu ustanovi da je kliničko stanje u poboljšanju, bez pojave drugih srčanih tegoba tipa poremećaja ritma ili novonastale zaduhe, ako su vrijednosti arterijskog tlaka zadovoljavajuće i vrijednosti lipidograma uredne, dolazi u obzir reduciranje doze lijekova te izostavljanje pojedine skupine lijekova (npr. prestanak dvojne antiagregacijske terapije nakon godinu dana), no naravno sve u dogovoru s kardiologom koji je upoznat s bolesnikovim stanjem.‘ rekla je dr. Dubravka Šušnjar.

‘Moj suprug je prije šest mjeseci imao infarkt miokarda. Uspješno mu je ugrađen stent. Uzima puno tableta u određeno vrijeme tijekom dana. Dobro se osjeća, osim što mu se pojavila erektilna disfunkcija. Prije infarkta sve je bilo u redu. Pretpostavljam da je to posljedica uzimanja određenih lijekova (za razrjeđivanje krvi).

Što je uzrok takve disfunkciji i jesu li to lijekovi koje uzima? Ruke i noge su mu hladne, mislim da to upućuje na lošu cirkulaciju. Hoće li se to moći popraviti i je li se s vremenom smanjuje broj lijekova kod takve dijagnoze?‘, upitala je gospođa iz Krapine, a odgovara dr. Dubravka Šušnjar, internist-kardiolog, KB Dubrava.

‘Budući da je vaš muž nedavno imao infarkt miokarda, uspješno je liječen perkutanom koronarnom intervencijom s ugradnjom stenta i nakon toga je uvedena medikamentozna terapija.

Uzimanje optimalne medikamentozne terapije nakon akutnog koronarnog sindroma potrebno je radi sprečavanja ponovnog ishemijskog događaja, poboljšanja srčane funkcije te poboljšanja preživljenja, stoga je nužno da se bolesnici toga pridržavaju.

Pri tome su antihipertenzivi najčešće izolirana skupina, a beta-blokatori i tiazidski diuretici prema brojnim studijama najviše povećavaju rizik od erektilne disfunkcije.

Točan mehanizam pojave erektilne disfunkcije uzrokovane beta-blokatorima nije sasvim razjašnjen. Smatra se da je to posljedica učinka na adrenergički sustav koji je uključen u fazu erekcije. Zanimljivo je da je jedna recentna studija pokazala da uloga periferne vazokonstrikcije kao posljedica uzimanja beta-blokatora ipak nema značajan učinak na erektilnu funkciju.

Ako se bolesnik pridržava svih savjeta liječnika, izbjegava čimbenike rizika za nastanak kardiovaskularnih događaja, uzima svu propisanu terapiju te se na kontrolnom kardiološkom pregledu ustanovi da je kliničko stanje u poboljšanju, bez pojave drugih srčanih tegoba tipa poremećaja ritma ili novonastale zaduhe, ako su vrijednosti arterijskog tlaka zadovoljavajuće i vrijednosti lipidograma uredne, dolazi u obzir reduciranje doze lijekova te izostavljanje pojedine skupine lijekova (npr. prestanak dvojne antiagregacijske terapije nakon godinu dana), no naravno sve u dogovoru s kardiologom koji je upoznat s bolesnikovim stanjem.‘ rekla je dr. Dubravka Šušnjar.

‘Moj suprug je prije šest mjeseci imao infarkt miokarda. Uspješno mu je ugrađen stent. Uzima puno tableta u određeno vrijeme tijekom dana. Dobro se osjeća, osim što mu se pojavila erektilna disfunkcija. Prije infarkta sve je bilo u redu. Pretpostavljam da je to posljedica uzimanja određenih lijekova (za razrjeđivanje krvi).

Što je uzrok takve disfunkciji i jesu li to lijekovi koje uzima? Ruke i noge su mu hladne, mislim da to upućuje na lošu cirkulaciju. Hoće li se to moći popraviti i je li se s vremenom smanjuje broj lijekova kod takve dijagnoze?‘, upitala je gospođa iz Krapine, a odgovara dr. Dubravka Šušnjar, internist-kardiolog, KB Dubrava.
